Please only complete the application after reading and agreeing to the accompanying policy below. There is a Word version to complete and print or a pdf one to print and fill in by hand.

Completed copies should be returned to the Cedars manager, Mrs Johnston, via the office prior to your child's starting date. Please also remember that we require advance bookings once registration has been completed.
